Patients may be referred to the Community Cardiology service from a variety of different avenues. Your cardiology condition may depend on which team you are referred to. Each individual team within Community Cardiology has referral criteria which depends on whether you are eligible.  

If you have recently been in hospital with a cardiology condition, you are likely to be referred straight to us directly.  

The referral criteria for Cardiac rehabilitation is a diagnosis of:

  • Heart attack and stent insertion 
  • Elective Angioplasty 
  • Heart valve surgery
  • Coronary Artery Bypass Grafting
  • Complex Device insertion 
  • Unstable angina (2x admissions in 6 Months)
  • Heart Transplant
  • Left Ventricular Assist Device
  • Chronic Heart Failure (If symptoms stable)

There may also be certain circumstances outside of the above mentioned which are discussed on an individual basis.
