Cardiac rehab does not have a set end date, and will conclude when you and the nurse both feel it is appropriate.
During the discharge call:
- your goals set during the initial consultation will be reviewed.
- the nurse will review your medications and any recent blood results/tests.
- together you will agree long term goals which mean you are likely to continue to make positive life changes long after Cardiac rehabilitation has finished.
After your discharge, the Cardiac rehab specialist nurse will inform your GP of any further ongoing plans you have made together.
You may receive a questionnaire once you have been discharged. We would be grateful if you could please complete this as it will help to shape the future of the service.
